Every caregiver has a unique story
Whether you’re a family member, friend or neighbor, Heritage’s Caregiver Specialists identify solutions for your unique caregiving needs and challenges.
Caregiver Counseling: assists you in making decisions and solving problems related to your caregiver role.
Options Counseling: provides guidance to make informed choices about long-term services and supports.
Caregiver Case Management: focuses on the caregiver to help support them during this important time. The service is made up of three key parts: 1) a comprehensive assessment, 2) development of a service plan to support the caregiver and 3) the coordination/monitoring of the services that make up the plan.
Information Services: provides information on resources and services in your community.
Supplemental Services: assesses needs and provides funding (on a limited basis) to complement the care you provide, such as home modifications, homemaker services, chore services, incontinence supplies and more.
Educational Sessions: from helpful tips to in-depth discussions, education on caregiving topics benefits both the caregiver and care receiver. Check the Events page for information on upcoming sessions.

Heritage also funds community resources
Adult Day Care / Adult Day Health: provides personal care for dependent adults in a supervised, protective and congregate setting.
Respite Care: services that offer temporary, substitute supports or living arrangements for care recipients to provide a brief period of relief for caregivers.
